• Proof of income everyone in the household (If you have no annual income, print out this form, click here, and notarize it before you start the application)

• Government-issued, photo identification (license, passport, etc.)

• Social Security cards for everyone in your home.

• Bank Statement

• Homeowners or Renters Insurance

• Deed OR Mortgage Statement

• Recent utility bills (PEPCO, Washington Gas, and DC Water)
